Attentive Home Health has teamed with its nurses and therapists to create customized wellness clinics for assisted and independent living facilities.

Our mutual objective is to develop disease management programs for the senior population. By integrating lifestyle and healthcare choices and optimizing social, physical and mental function, we can eliminate barriers to unmet potential.

Through education and information, Attentive Home Health provides seniors and their families with the necessary clinical support to live in place longer and more independently than at anytime previously.

Health Education Clinic

• Monthly Disease Management Programs
• Early screenings for illness, injury and emotional well-being
• Nurse Consultation regading medications, health conditions and when to call a doctor

Bodies in Motion Clinic

• Monthly Exercise Program (a physician release is required for participation)
• Falls and Balance Assesment
• Home Safety and Home Adaptations
• Free In-Home/Apartment Assesments

Coordination of Care
Upon request, a nurse will contact a participant's doctor to report any health concerns.
